Pretty much everyone who can afford it has insurance because there is an individual mandate enforced by the Department of Revenue. However, the insurance companies still act as gatekeepers to health care, and their decisions are made with their profit margins in mind, not with anyone's health care needs. It's a system only Mitt Romney could love.
